If you have a complaint about a WIC grocery store / vendor, a WIC participant, a WIC staff member, or any other DC WIC-related matter, please fill out the form below.
The more information you can provide, the better. For example, if your complaint is about a grocery store, please include the store's name and location (if more than one location in DC), the names of any staff involved in the incident, and a detailed description of the incident (date, time, issue, etc).
Once a complaint is received, a member of the DC WIC State Agency will follow up with you within three (3) business days.
As an alternative to filing a complaint online, WIC staff, participants, and vendors may print the following forms and submit to the State Agency via email to info.wic@dc.gov
